Social Determinants of Health: The Path Forward for Actionable, Interoperable Data to Improve Health

Our February 19 meeting, which was a Zoom call, focused on health system preparedness. The topic of the meeting was: Social Determinants of Health: The Path Forward for Actionable, Interoperable Data to Improve Health. The topic of Social Determinants of Health (SDoH) data has evolved considerably over the last few years, and even gained in importance during the current pandemic. While many in the industry now have a general understanding of what SDoH data is, this session explored current and future pathways for collecting, standardizing, sharing and analyzing the impact of that data on individual and population health. Our speakers included: (1) Ashish Kachru, co-founder and President of Altruista Health, a HealthEdge company. Altruista Health was built from the ground up with a special focus on safety net populations, and features the GuidingCare care management platform as a comprehensive solution that integrates all preventive, acute care, chronic disease management, behavioral health, long-term care and community services into a one seamless, patient-centered model of care. (2) Evelyn Gallego, Founder & CEO at EMI Advisors LLC, and SIREN/ HL7/Gravity Project Program Manager, who provided an overview of HL7’s Gravity project and discuss the project’s roadmap and achievements toward making SDoH data interoperable. The Gravity Project’s mission is to create and maintain a consensus-building community to expand available SDoH core data for interoperability and accelerate standards-based information exchange by using HL7® FHIR®.
